Necrotic Heart is a Redstone item dropped by Vampire King in The Vampire Manor. It is a unique drop, not a piece of the King class-loot equipment selection, and its listed chance is 100% on Normal, Hard, and Mythic.
That makes this the straightforward reward in a fight full of less certain equipment drops. You still have to defeat the Vampire King; the heart is tied to the King’s final phase, after the encounter has passed through its earlier health thresholds.
Its flavor text reads “A heart that never sleeps, pulsing with ancient malice. Its relentless rhythm echoes the will of a dark king long unchallenged.” The name sounds like trouble, but the item is listed as Redstone rather than a weapon or armor piece.

There is no stronger Heart variant to chase by changing the dungeon difficulty. Hard and Mythic change the encounter, but this particular unique item keeps the same listed chance and material.
Where to Get It
| Dropped By | Chance | Difficulty | Notes |
|---|---|---|---|
| Vampire King | 100% | Normal | Final phase; one item |
| Vampire King | 100% | Hard | Final phase; one item |
| Vampire King | 100% | Mythic | Final phase; one item |
The King begins as an Evoker and changes to an Iron Golem for its later phases. Its transitions occur at 99.99%, 80%, 60%, 40%, and 20% health; the Heart belongs to the loot-bearing last phase rather than the opening phase.
Getting through those phases is the real obstacle. The King resists player projectiles and knockback, gains its claw attack in phase two, and later adds a gaze attack that blinds and pushes players out of position.
Is It Worth It?
If your goal is specifically the Necrotic Heart, Normal is the practical first choice: the listed drop rate does not improve on Hard or Mythic. Higher difficulty is worth considering when you also want the King’s better equipment, not because it gives the Heart a higher chance.
Its value here is as a reliable, non-soulbound unique collectible from the final boss. No combat enchantments or potion effects are attached to the Heart, so judge it for what it is rather than expecting hidden armor-grade bonuses.
