Dusty Necklace is a unique Lead dropped by Enraged Spirit in The Vampire Manor. It has a 33% drop chance on Normal, Hard, and Mythic, so there is no harder-difficulty version you need to chase for this particular item.
This necklace is covered in dust.
The item is tied to an enemy encountered outside the manor and connected to the Village Ruins! quest. It is a straightforward named drop rather than a weapon with a long list of extra combat effects.

Farming the Enraged Spirit
Enraged Spirit has Attack Push and permanent Fire Resistance on every difficulty. On Mythic, it gains Taze, which can jolt and heavily slow players when it hits them. None of those additions increases the necklace’s listed drop percentage.
If the necklace is your only goal, Normal keeps the item’s 33% chance without Mythic Taze. If you are also progressing Village Ruins!, the same enemy counts as a kill objective for that quest.
Is It Worth It
Dusty Necklace is worth pursuing if you want this named manor drop or are already fighting Enraged Spirits for the quest. Its value lies in the collectible and encounter connection, not an enchantment or potion effect: the necklace has none listed.
